Any Qualified Provider for Audiology Services for Gateshead and South Tyneside - This Any Qualified Provider for Audiology Services is being commissioned by North of England Commissioning Support Unit for and on behalf of: NHS Newcastle Gateshead Clinical Commissioning Group; and NHS South Tyneside Clinical Commissioning Group. GB-Leicester: Independent Advocacy Service for People with Mental Health Issues Targeting the African, African Caribbean & Asian Communities - The Council is seeking a provider of Independent Advocacy Support who can meet the specific needs of those with Mental Health issues from within the African, African Caribbean and Asian communities in order to ensure customers are supported by workers with the relevant skills and experience whilst at the same time, supporting the Council to understand the impact of the Care Act in relation to advocacy provision for this group. The proposed Contract will be for seven months with an option to extend for a further period of 5 months and it is anticipated to award to a single Contractor. The delivery of an Independent Advocacy Service to adults (aged 18+) and older people, plus young people in transition (people preparing for adulthood) resident in Leicester City who are vulnerable, disabled or have complex needs and who receive, are being assessed for, or are entitled to receive services from the Council’s Adult Social Care department. The service will be provided to eligible adults who advocacy need support to enable them to be heard, to secure their rights and get services they need.

The service provider(s) shall: • Mitigate the practical, emotional, occupational or educational impacts of individuals’ sight loss. - Patients’ satisfaction with the outcome of service. - As the service will be commissioned under AQP arrangements, payment will be made at national tariff for consultant-led care; there is no guarantee of income. The provider has full responsibility for the costs of any drugs used/prescribed within the service. Rapid Access to Carers at End of Life Plus (RacePlus) - The Rapid Access to Carers at End of Life (RACE) Plus will provide direct patient care at end of life. To support a patient and their carer in their home or care home setting, for a maximum of a 48 hour period or 72 hour period over a bank holiday period, during what is perceived to be the final six weeks of the patient’s life or for a patient with a life limiting condition when their caring situation is considered untenable by a health care professional until an appropriate care package can be arranged. This will enable people to die or be cared for in their home setting if is their preferred choice of care, maintaining continuity of care and preventing unnecessary hospital admissions. The overarching aim of the service is to provide a high quality service in end of life care, responding to demand and delivering prompt end of life care on the day of referral via a single point of access with staff trained via an evidenced based End of Life Care programme.
