CBC-1641-T-LS Home Recovery Service - The Home Recovery Service is designed to be holistic in nature intended to support patients to recover from hospitalisation; to recuperate. The service will remain dynamic, delivered in a way that ensure flexibility to deliver what is necessary across the entire service. The service will foster a culture of independence, working with people to gain confidence with ordinary tasks of daily living. Call times will be agreed with person leaving hospital; specific calls for medication or management of skin integrity will be prioritised. HRS is one 'whole' service that is intended to be delivered by two providers. Having two providers ensures that risk to delivery is minimised. As such, providers are expected to work closely together, share good practice, share challenging periods in delivery to maintain the rate of discharges required, work to resolve complex situations and / or scenarios and most importantly put people in need of the service at the forefront of planning and every intervention; providers will be expected to work well in collaboration and support each other's service delivery & continuity should the need arise.

Hospital Discharge Service - To extend the current provision of the Hospital Discharge service, that was compliantly procured, for a further 12 weeks from 1st April 2023 to 25th June 2023 to allow a full review and evaluation of the service to take place. Following the review and evaluation options will be considered to further extend the service, end or re-procure - with the appropriate governance in place. The Hospital Discharge service will be procured at the uplifted 23/24 rate of £26.60 per hour - 252 block hours per week - a total approx. weekly cost of £6,700 - 12 weeks total cost approx. £80,500. The service cost to date is approximately £82,000 (December 2022 to March 2023).

A 1 year contract with the option to extend for a further 1 year (2 Years in total) This is for a new, Rough Sleepers and those a Risk of Rough Sleeping community outreach drugs and alcohol services. The aims of the service are to deliver harm reduction, engagement to structured treatment and recovery of substance us for Rough Sleepers and those at Risk of Rough Sleeping. The outcomes are to reduce drug related deaths, increase numbers into structured treatment, reduce homelessness and sustain tenancies. The provider of Lot 1 will be required to bid for Lot 2 to ensure a strengthened and single seamless service offer
