NOTICE CLOSED - Specialist Physiotherapy - NHS Bracknell and Ascot Clinical Commissioning Group - NOTICE CLOSED - NHS Bracknell and Ascot Clinical Commissioning Group (CCG) have been running a successful pilot scheme where specialist physiotherapy is used in a structured course of conservative management for patients who are otherwise indicated for total knee replacement. The scheme uses the principles of implicit motor retraining in order to restore a biomechanical equilibrium and offset load through the arthritic joint. The service also utilises techniques from other fields, such as motivational interviewing and Cognitive Based Therapy (CBT) to provide individualised programmes. This conservative management is not routinely funded on the NHS. It is an innovative pathway with a unique treatment methodology different to the current mainstream pathways which target segmental strengthening and flexibility programmes. The pilot has demonstrated that the programme can achieve outcomes comparable to those achieved with surgery which are sustained for at least 12 months. Clinical audit has confirmed that more than 75% of patients achieve reductions in pain, stiffness and improvements in functionality, enabling patients to avoid surgery for more than 1 year. There is a need to establish the service on a more formal basis with the appointment of a single service provider, with evidence based experience in reducing total knee replacements (TKR) by over 75% through conservative management as described above.
